• Photo Requirements:  Please prepare two photos, each sized 6 cm × 6 cm (one for the Astraea measurement and one for the Kazutama information transfer). Copies are acceptable.
  Please write your full name on the front of the photo, and your date of birth and address on the back.

  A simplified measurement of both parents is also required alongside the individual’s measurement.
